QUALITY IMPROVEMENT TOOLS AND TECHNIQUES

Cause-and-Effect Diagram ( Fishbone ) 1

Definition : A diagram designed to help teams picture a large number of possible causes of a particular outcome.
   
Purpose :

Help a team

  • identify and define an outcome or a problem.
  • determine causes of a given outcome or problem.
  • identify causes for variation in a process.
   
Procedure :
  • Place the outcome (or problem statement) on the right side of the paper, halfway down; draw a horizontal line across the paper with an arrow pointing to the outcome.
  • Determine major categories for the causes; connect them to the horizontal line with diagonal lines.
  • Note the main causes and place them under the major categories.
  • Try to list subcauses and place them under the main causes.
  • Evaluate the diagram.
