A diagram designed
to help teams picture a large number of possible causes of a particular
outcome. |

| Purpose
: |
Help a team
- identify
and define an outcome or a problem.
- determine
causes of a given outcome or problem.
- identify
causes for variation in a process.

|
| Procedure
: |
- Place the
outcome (or problem statement) on the right side of the paper,
halfway down; draw a horizontal line across the paper with an
arrow pointing to the outcome.
- Determine
major categories for the causes; connect them to the horizontal
line with diagonal lines.
- Note the
main causes and place them under the major categories.
- Try to list
subcauses and place them under the main causes.
- Evaluate
the diagram.
