QUALITY IMPROVEMENT TOOLS AND TECHNIQUES Cause-and-Effect Diagram ( Fishbone ) 1 Definition : A diagram designed to help teams picture a large number of possible causes of a particular outcome.     Purpose : Help a team identify and define an outcome or a problem. determine causes of a given outcome or problem. identify causes for variation in a process.     Procedure : Place the outcome (or problem statement) on the right side of the paper, halfway down; draw a horizontal line across the paper with an arrow pointing to the outcome. Determine major categories for the causes; connect them to the horizontal line with diagonal lines. Note the main causes and place them under the major categories. Try to list subcauses and place them under the main causes. Evaluate the diagram.
