Determinants of Health Services Utilization of the Chinese Adult Population in Hong Kong

The study assessed the role of chronic morbidity and health-related quality of life which was measured by the SF-36 scores as determinants of service utilization for the Chinese adult population in Hong Kong. Both of them were stronger determinants than sociodemographic variables.

The medical consultation pattern of school children in HK was analyzed. It was revealed that school children had high rate of consultation and self-medication, and the health status of older school children is not better. They were not offered adequate preventive advice. The piecemeal approach to focus on curative medicine will only lead to high rate of episodic consultation and self medication without empowering the young people the skills of self care and self help, and positive health behavior.

The health concepts that wee thought to be most important were physical functioning and mental status, daily role and social functioning were also important. Life habits, the environment and emotions were most commonly identified as factors that could affect health. Health was thought to affect daily role functioning and activities the most.

Chronic diseases are very common and not restricted to elderly. The chronically ill's quality of life may be further jeopardized if they have to pay for their medical care at the expenses of other daily needs.

To find out how people determined quality of a doctor's consultation. Although cost was considered to be very important, there were other items perceived to be even more important, including having the doctor explain the condition to a patient, having the doctor listen carefully to a patient, the chance to learn more about health and seeing the same doctor each time.
   
7. Patients' Attitudes Towards Medicines in Different Family Practice Settings in Hong Kong: Is the Family Physician Handicapped by the Health Care System?

The study compared patients' knowledge, attitude and practice (KAP) of medicine use among different family practice settings. Variations in patients' KAP on medicine use among family practices were related to healthcare system factors (e.g. payment method and accessibility), patient-factors, doctor-factors and the doctor-patient relationship. Family physicians appear to influence their patient's view in different healthcare settings. Supportive practice systems that foster long-term patient-family doctor relationships and encourage greater information exchange can augment their effectiveness.

The high level of inappropriate utilization of A&E service indicating problems of delivery of GP services in HK. Convenience, accessibility and perceived better quality of services and urgency for help rather than cost are the main factors. The GPs should develop a system to provide out of hours services maintaining continuity of care.

10. Utilization of Primary Medical Care Service by Chinese Patients Diagnosed with Psychological Disorders Compared with Age-Sex Matched controls over a five year period

Patients with psychological disorders had greater utilization of family physician services. Patients with somatoform disorders receiving continuing care had similar visit rates as controls and did not have to present with more physical symptoms in order to see the doctor.
